Registration Form

The application is meant to print on 8 1/2 " x 14" paper.   After selecting the Print icon, check "Choose Paper Source by PDF page size" for 8 1/2" x 14" paper. 

The application will print compressed on 8 1/2 x 11 paper, but it is more difficult to read and you must write smaller.
